For those living with chronic illness; quality care and comfort are essential. Pine Haven collaborates with local palliative care providers and has a preferred relationship with the Sharon S. Richardson Community Hospice.
Palliative care is specialized medical care that focuses on providing relief from pain and other symptoms for those living with an advancing chronic illness. Our Giddings Avenue campus is located at 531 Giddings Avenue in Sheboygan Falls, just north of Highway 28 on Highway 32.
Our Oostburg campus is located at 701 Pine Drive in Oostburg, at the corner of North 12th Street and Holland-Lima Road.
Our Prairie Crossing campus is located at 1280 Pine Haven Ln, Sheboygan Falls, just south of Highway 23 and west of Highway 32.
